US Candidate Applicant Statement

I authorize the Company to obtain information regarding my former employment, education, credit, driving, civil and criminal background. I hereby release the Company, its affiliates, officers, directors, employees, representatives and agents from any and all liability, including liability arising from the Company's negligence, arising from the Company's verification of my prior employment history, criminal record, references and any other background information pertaining to me. I also release from any and all liability, including liability arising from negligence, all persons and entities who supply the Company with information pertaining to my prior employment history, criminal record, references and any other background information pertaining to me. I understand that all answers provided by me on this application and all statements made by me during the application process, including statements made during the post-offer physical examination, are open to investigation by this Company, and that if any information given by me in this application process is found to be false or misleading, I will be subject to dismissal at any time during the period of my employment, and I agree to hold the Company, its affiliates, officers, directors, employees, representatives and agents harmless in that event.

I further understand that any offer of employment made by the Company is subject to the receipt of satisfactory references from former employers. If employment is obtained, I will comply with all rules and regulations of the Company. I agree to be responsible with Company property and equipment issued to me by the Company until returned. I agree to submit to a physical examination and drug/alcohol testing, including random testing at any time, as required by the Company. Further, I hereby understand and agree that my employment is at will, that nothing in this application or in any other Company document shall be deemed to create any contract of employment between me and the Company, and that my employment can be terminated at any time by me or the Company for any reason, or for no reason, with or without cause or prior notice. I understand and agree that any statements to the contrary, whether oral or written, will not alter the employment-at-will relationship in any way. I further understand that no representative of the Company, other than the President of the Company, has any authority to enter into any agreement for employment for any specified period of time or to make any agreement, oral or written, which changes or otherwise affects the employment-at-will relationship. If employment is obtained, I will comply with all rules and regulations of the Company.
